If possible, please complete the following information prior to our meeting. This information is required for us to complete your loved one’s death certificate. Since this is a legal record, we want to make sure that all the information is accurate. It is very difficult to make changes after the certificate is filed, so we want to make sure that everything is perfect. If you don’t know all the information at this time, please put a “-” in the field. You can always let us know later.


If you have not notified the funeral home yet about the death of your loved one, please call us before completing this form.


**For veterans, we have access to a password protected VA database. If you are unable to locate your loved one’s military discharge documentation at this time, we may be able to access it online for you.


Keep in mind, this information is just for the death certificate and will not be published anywhere. We will discuss obituary information and biographical details when we meet.
